Description
Chart dividers are designed for maximum organization within a file folder. They provide quick and easy referencing for access to pertinent information in an efficient manner and are a great time saver. Our index sets consist of 8, 6 or 4-chart dividers with Mylar colored tabs. Tabs feature popular titles plus 1- write on blank, erasable tab designed for your specific needs. All tabs are printed with the same title on both sides and reinforced with tear proof Mylar.

These stock medical chart dividers include 100 sets of 4 divider sheets with pre-printed, 1/6th cut side tabs.
Each sheet is 8-1/2" x 11" with a the mylar laminated tabs extending an additional 1/2" on the side.
These divider sheets are made out of a lb. (90#) manila stock and tabs are printed in black ink on both sides. Each sheet has five, mylar reinforced, holes punched on the side and two holes punched on the top.
